A destructive confirmation dialog naming the change order number, warning that the builder will be notified and the change will not go ahead, with a mandatory reason field of at least three characters. Confirming closes the whole document modal and flips the card's status pill to Declined.

A change order that comes back declined with no explanation just becomes a phone call, and the builder is left guessing whether it was the price, the timing or the scope. Declining asks for a reason and records it against the order. The builder answers the actual objection instead of re-pitching the whole thing blind.
